Return & Cancellation Policy 

Return your eligible item within 7 days of receipt in its original packaging and condition to receive full refund. Please email info@formaliving.com with your order number and reason for return and a representative will provide you with a Return Authorization Number within 2 business days. Please note, returns will NOT be accepted without an RA#.

Return your eligible item within 8-21 days of receipt in its original packaging and condition for Forma Living store credit. Please email sales@formaliving.com with your order number and reason for return and a representative will provide you with a Return Authorization Number. Please note, returns will NOT be accepted without an RA#.

Write the RA# on your invoice and include with your return. Clearly, mark the outside of the return box with the RA#.

Pack your return securely in original box and original packing material and be sure to ship it via insured or certified US Mail or UPS Ground. Insuring or certifying your US mail shipment allows for you to be protected should your return not arrive at our warehouse.  Retain your shipping receipt for your records. 

Refunds are issued in the original form of payment, and do not include shipping and handling charges, applicable re-stocking fees and return shipping charges. 

Damages

Please keep original packaging and document both the item and packaging damage with pictures. Claims against damaged items must be made within 48 hours of receiving the item. Claims made after 48 hours cannot be accepted. Inspect your delivered item before you sign to ensure there’s been no freight damage.
